Obverse: in a pearl border decorated with labrys, a four-field shield with coats of arms: Chalecki, Rola, Korczak and coat of arms unknown, in the jewel a winged arrow, at the edge a linear border, in the rim: DEME CHAL DE - CHALCE TES M D L.
Reverse: In the rim the inscription: .1.5.9.5. / BENEDICTIO / DOMINI . DIVI. / TES . FACIT, below it an ornamental floral motif, and below it a monogram HD.
